Do not wait until an evacuation order is issued -- when an emergency arises, pre-registered individuals will have priority over those who have not registered for the program.
Registrants will be contacted on a semi-annual basis to re-certify their need for this program. Registrants do not need to complete an application every year, but it is the registrant's or caregiver's responsibility to report any changes to the information provided on the application by calling EEAP's Support Line at 305-513-7700 or Miami-Dade County's 3-1-1 Answer Center by dialing 3-1-1.
To apply for yourself or on someone's behalf, complete all sections of the application below, or apply online by clicking here.
If more than one person requires medical assistance during evacuations, each person must complete and submit a separate application. Additional copies of the application can be obtained in the mail by contacting Miami-Dade County's 3-1-1 Answer Center or download online at http://www.miamidade.gov/fire/eeap.asp. If the applicant requires a higher level of medical care, their primary care physician should complete and sign the application before submission.
Office of Emergency Management (OEM) or support personnel will contact you to verify your registration information and answer any questions. Applicants will receive a letter informing them if they have been accepted into the program. This letter will include the evacuation assistance they are eligible to receive and their responsibilities.
To request and application in English, Spanish, Haitian Creole or in an alternate format such as Braille, large print or electronically, contact the Miami-Dade 3-1-1 Answer Center or calling (305) 468-5900 or toll-free at 888-311-DADE (3233).
